KEYS Membership Plan

 Affordable Dental Care with our In-House Membership Plan That Works for You

At Greenfield Dental Health Care, we believe everyone deserves access to high-quality dental care—even without traditional insurance. That’s why we offer our KEYS Membership Plan to help individuals and families manage their dental costs with ease and confidence.

What Is It?

Our KEYS Membership Plan is an in-house membership-based program that offers significant savings on preventive and restorative dental services—all for one low annual fee. It's not insurance, but it works like a discount program to provide consistent, affordable care. The plan year runs for 12 months and renews on the first of the month in which you originally enrolled. You’ll receive a reminder before your renewal date to ensure uninterrupted benefits.

Plan Benefits:

Prophylaxis (Routine Care) Plan

Designed for patients with healthy gums and no history of periodontal disease.

Annual Membership Includes:

  • 2 cleanings (prophylaxis)
  • 2 exams (includes oral cancer screening)
  •  Annual bitewing x-rays
  • Fluoride Treatments
  • 1 Emergency/Problem focused exam with PA X-ray 

Periodontal Maintenance Plan

Designed for patients with a history of periodontal disease who require ongoing maintenance cleanings.

Annual Membership Includes:

  • Up to 4 periodontal maintenance visits per year (as recommended by your provider)
  • Up to 4 exams (includes oral cancer screening)
  • All necessary X-rays (including annual and progress evaluations)
  • Periodontal charting as needed to monitor pocket depths
  • Fluoride treatments

Any treatment not specifically listed under the Routine Care or Periodontal Maintenance KEYS Membership Plans is still eligible for valuable savings:

  • 25% off Preventive & Diagnostic procedures (e.g., sealants, Panoramic X-rays)

  • 15% off Restorative & Reconstructive procedures (e.g., fillings, crowns, bridges, dentures, implants)

  • 10% off Orthodontic & Cosmetic procedures (e.g., clear aligners, veneers, whitening)

These benefits and discounts apply exclusively to services provided by Greenfield Dental Health Care and are available only to current KEYS plan members.
